At diagnosis, T1D in children is associated with various medical, psychological, social, and organizational hurdles. In the hospital, families are often challenged with a lack of a cohesive approach to care. This session presents the application of a multidisciplinary, patient/family-centered model for patients with new-onset T1D used at Texas Children’s Hospital for 10 years. Families are assessed and educated by a team with defined roles: physicians, nurses, CDCES, pharmacists, dietitians, psychologists, and social workers. The team works together to support the educational and emotional needs of the patient and their caregivers. With appropriate, supportive medical care and education at diagnosis, children and caregivers can gain the skills, knowledge, and confidence needed for safe diabetes self-management at home.
