(P217) Diabetes Demonstration Project: Increasing Access to DSMES for a Medicaid Population

The Diabetes Demonstration Project was a collaborative effort between the DC Department of Health (DC Health), Medicaid Managed Care Organizations (MCOs), and Sorogi Inc. This poster reviews a year-long project funded by DC Health with the goal of increasing access to DSMES for Medicaid enrollees living with diabetes. The MCOs engaged were required to submit direct referrals to the Sorogi DSMES program. Of the direct referrals received, over 75% of eligible enrollees were African American or of Hispanic descent and of low-income status. The project used an active remote-patient-monitoring program and weekly educational sessions to bridge the gap between medical visits.
