(P233) New Resources to Engage Advanced Practice Providers in Referrals to the NDPP Lifestyle Change Program

Referrals from health care providers are important for getting those at risk for T2D into the National Diabetes Prevention Program lifestyle change program (LCP). Advanced practice providers often spend more time with patients, are trained to provide health education, and focus on preventive care. This poster describes a study to explore advanced practice providers (NPs, PAs) as a referral source—and how LCP organizations could engage them through formative research with NPs, PAs, and LCP organization recruitment staff. The findings informed the development of a suite of outreach tools and materials for LCP organizations to use to engage advanced practice providers in referrals. The materials were further evaluated through a demonstration project with four LCP organizations.
