(P323) Use of Roundtable Panels to Reduce Gaps in Care and Improve A1C in Patients with Diabetes

Despite an improved understanding of the disease state, screening initiatives, and pharmacological/technological advances, significant gaps remain in delivering effective diabetes care. Less than one-quarter of people with T1D and less than half of people with T2D meet the American Diabetes Association’s recommended hemoglobin A1c (HbA1c) goal of < 7.0% for most people. This poster describes our aim to improve care for employees and their dependents with an HbA1c >8.5% enrolled in our health plan and how we instituted multidisciplinary roundtables that aided in diabetes self-management. We further describe how the interventions effectively utilized resources across the health system, without placing additional load or burden on primary care or diabetes specialty care teams.
