Beaufort Jasper Hampton Comprehensive Health Services (BJHCHS) added diabetes case management and 2 community health workers in July 2022 after receiving a grant from Diabetes Free South Carolina /Blue Cross Blue Shield. In addition, they added remote glucose patient monitoring and comprehensive medication review with a pharmacist. This poster describes the A1C outcomes of adding diabetes case management, remote patient monitoring, and comprehensive medication reviews to their services.
