(P321) The Use of Innovative Practices to Improve Diabetes-Related Neuropathy Screenings

Unmanaged diabetes can result in long-term complications like foot amputations. With the recent global pandemic there has been inconsistent follow-up and an increase in telehealth visits over the past few years. The purpose of this intervention was to address overdue foot exams at the Family Health Center of Harlem by increasing medical office assistant (MOA) performance through engagement. In this poster, review the tools and strategies implemented to improve neuropathy screenings at an FQHC, how to engage with interdisciplinary staff, and ways to apply this to other patient-centered practices.
