(P408) Implementing a Diabetes Distress Screening Tool to Tailor Diabetes Self-Management Education and Support

The daily demands of diabetes self-management are associated with emotions sometimes referred to as diabetes distress, where patients report feeling distressed about their diabetes management. High diabetes distress can impact the ability of an individual with diabetes to reach their targeted glycemic goal. About 25% of individuals with diabetes have experienced diabetes distress which is underreported, and prevalent in 8.9% of hospitalized patients compared to 1.2% of patients seen in primary care. This poster provides a synopsis of how a Level 1 Trauma, Magnet hospital in Ohio, implemented an evidence-based practice tool (DDS-17) to screen patients for diabetes distress. DDS-17 is a validated 17-item questionnaire with the following categories: emotional burden, regimen, and interpersonal and provider-related distress experienced over the past month. The first phase of this project consisted of diabetes care and education specialists educating patients on diabetes distress and assisting patients to complete the DDS-17 questionnaire. The second phase involves using the 7-Step Problem-Solving Treatment approach to tailor patients' diabetes education and support to help them reach their glycemic targets. This project aligned with the American Diabetes Association (ADA) standards of care pertaining to emotional support and individualized patient care.
