(P409) Integrating Behavioral Health to Support People Living with Diabetes

Incorporating a Behavioral Health Integration Model can support the healthcare team to make the best use of their resources and can support the patient by providing comprehensive care all in one place, and by reducing barriers to accessing mental health care, including transportation and stigma. This poster reviews the benefits and challenges of Behavioral Health Integration (BHI) approaches in an interprofessional diabetes clinic that serves uninsured patients, who are often naive to navigating healthcare systems and may be less likely to pursue mental health treatment outside of the diabetes clinic. Resources will be shared to assist other practices in adopting a BHI model of care.
