(P503) CDCES Driven Innovation to Improve Care of Patients with Gestational DIabetes

Women with gestational diabetes are more likely to have pre-term births and pregnancy complications. In the US, 8% of pregnancies are affected by gestational diabetes (GDM). This rate has increased by 30% over the past 5 years and the incidence of GDM is higher in communities of color. Access to specialized care is challenging. This poster describes a partnership between Stability Health and an obstetrics practice located in a multi-specialty group serving an urban, underserved population to provide access to gestational diabetes education and support plus provide endocrine expertise to the OB providers through a tech-enabled model that combines human-centered coaching with evidence-based care built into a technology platform.
