DSMES can be used as part of skilled care for persons with diabetes (PWDs) in home health; however, it is not always obvious that DSMES can be defined as skilled care. As a result, DSMES is under-utilized in home health. At CenterWell Home Health, we deliver DSMES as part of the American Diabetes Association Education Recognition Program (ADA-ERP). This poster will review data related to interprofessional care team members who provide DSMES, visits per episode by role, and will link this information to outcomes. We will identify gaps and opportunities to optimize the delivery of DSMES in home health.
