Challenges facing emerging adults with diabetes has been well documented in the literature. Issues with worsening glycemic management and lack of follow up within this age group has been confirmed due to various factors, such as gaps in knowledge, lack of transition preparation, and differences in the perceived pediatric/adult environment. Our clinic implemented a new transition program in 2022 using the six core elements of healthcare transition. This poster highlights the top 3 knowledge gaps in a pediatric cohort of racially diverse adolescents that were addressed through educational sessions highlighting important transitional topics.
