The amount of health information that streams across devices or in conversations could be overwhelming to anyone, but for a person with diabetes, this is only intensified. Diabetes care and education specialists are poised to break down needed care into more understandable and actionable strategies that meet the person with diabetes where they are. This poster highlights the refreshed CDC DSMES Toolkit and a new resource for delivering DSMES services to individuals with varying levels of health literacy and English proficiency. From the mindset check-in to the visual aid and plain-language libraries that correspond to the ADCES7 Self-Care Behaviors, the poster authors will help explore these tools and glean practical approaches to incorporate them into existing efforts.
