(P211) Community-Based Organizations Preventing T2D in Rural Counties of the Diabetes Belt

In Alabama, diabetes was the seventh leading cause of death in 2019 with 13.9 percent of AL’s adult population reporting they have been told they have diabetes. Diabetes is especially prevalent in rural areas considered part of the US Diabetes Belt. This poster will examine lessons learned from three Alabama-based organizations that worked with the Black Women’s Health Imperative to establish the National Diabetes Prevention Program in the rural communities of Montgomery, Selma, and Tuscaloosa, Alabama. Two federally qualified health centers and a non-clinical, community-based organization share how programs were established and how they continue to recruit and retain participants living in Alabama Diabetes Belt communities.
