(P314) Increasing After Visit Summary Documentation of Post Hospital Follow-up Appointments

Hospitalized individuals with diabetes are at two times greater risk for readmissions than those without diabetes. Evidence shows that in collaboration with the person living with diabetes, pre-scheduling a follow-up appointment with a primary provider or endocrinologist prior to discharge can increase the likelihood that a post-hospital follow-up appointment will be attended. This poster describes a multidisciplinary, quality improvement effort to increase the percentage of pre-scheduled post-hospital follow-up appointments. The team's effort to facilitate the transition of care during discharge expands communication and partnership among health care professionals, as well as, decreases the individual’s burden of scheduling a post-discharge appointment once leaving the hospital.
