(P100) A Population Health Approach to Implementing DSMT Services in a Hospital-Based Setting

Suburban Hospital's Diabetes Self-Management Training Program has truly opened the eyes of its participants. This poster describes how targeted and individualized sessions with a certified diabetes care and education specialist led patients to feel more in control of their diabetes care. They stated that they "never knew they were doing everything wrong" and "feel like it is all going to be ok". Through this program, Suburban has been able to report a decrease in hospitalization costs for this population, even after attending only one session with the educator.
