(P407) Implementation of an Assessment and a Curriculum to Reduce Diabetes Distress in a Primary Care Setting

Distress Distress (DD) refers to the negative emotions and stress of living with diabetes. This distress can negatively impact coping and ability to self-manage diabetes leading to higher A1C levels and over time, and higher rates of long term complications. Early screening to address DD and behavioral health referrals may improve diabetes outcomes in the long term. This poster describes an initiative, with a goal to assess and identify at least 90% of patients with diabetes for distress, so that they could be referred and treated appropriately. The objective was to provide distress specific education to those in need.
