(P525) Telehealth is Changing the Way We Manage T2D

Diabetes self-management can be challenging, particularly for patients who are socioeconomically disenfranchised, stigmatized and lack legal status. This poster discusses how a team of diabetes educators employed by a metropolitan safety-net healthcare organization conducted an evidence-based practice study to determine the impact of diabetes education delivered via telehealth in improving hemoglobin A1c. The poster authors also share data-supported evidence regarding facilitating, impeding, and mitigating factors related to telehealth interventions.
