(P209) Care Coordination Initiative for Complex Patients with T1D and T2D

The Kovler Diabetes Center at the University of Chicago added a dedicated social worker to its team to help deliver care to its population of all ages and diverse backgrounds. This poster gives examples of using multidisciplinary communication and collaboration to provide complex case management and care coordination in an outpatient endocrinology clinic at an academic medical center. The poster authors share a template for how to coordinate care in complex patients with T!D and T2D and how to connect patients with helpful tools to improve diabetes management and quality of life.
