Hyperglycemia in hospitalized patients is associated with adverse clinical outcomes and longer lengths of stay. It is estimated that one-third of hospitalized patients will experience significant hyperglycemia. Minimizing hyperglycemia while avoiding hypoglycemia in this setting is challenging because of obstacles such as acute illness, inconsistent caloric intake, and changes to medications different than what the patient was taking at home. This poster outlines a pharmacist-led inpatient stewardship program implemented at Cooperman Barnabas Medical Center in October of 2022. The program includes a pharmacist consultation service for adult inpatients with diabetes and renal dysfunction, targeted pharmacist interventions to prevent hyperglycemia, and pharmacist recommendations for medications at discharge.
