The Kentucky Diabetes Network created a campaign titled “Prevent Type 2 in ‘22” to increase awareness and utilization of diabetes prevention and health promotion programs and services in Kentucky. This poster outlines the goals, components, and outcomes used to reach and support Kentuckians who are at risk for developing T2D, as well as connect those at risk with health and wellness providers throughout the Commonwealth.
