F04 - Provide Whole-Person Care Through a Virtual Diabetes Specialty Clinic

Fully remote diabetes care with diabetes technology integration and support leads to improvements in diabetes self-management and health outcomes. Incorporating a behavioral health intervention as part of overall diabetes care has a significant impact on improving overall diabetes management and diabetes distress. This session describes how a virtual clinic team was able to deliver personalized wellness and diabetes-related mental health support as part of a whole-person care model. The presenters will share how they introduced CGM technology into diabetes self-management, used decision support technology, and adjusted medications and treatment plans via a virtual platform to increase access to specialty care without limitations related to geography and cost.
