Equitable access to diabetes services is a public health issue facing economically and socially marginalized communities. Those individuals disproportionately affected by the complexity of diabetes struggle with social determinants of health, resulting in limited participation in diabetes self-management. New York State developed an innovative approach to expanding diabetes care by using community pharmacists, who are accessible, yet underutilized, community-based health care professionals. A clinically integrated network of community pharmacies expanded access to diabetes self-management, increased rates of disease state screening, and created low-literacy educational materials to optimize patient outcomes resulting in increased levels of service utilization. This approach established a pharmacy value proposition and created sustainable pathways to expand education in community pharmacies.
