The Veterans Health Administration is the largest integrated healthcare system in the world serving veterans in both urban and rural environments. There are more than 9 million veterans enrolled in the VA system and 1.6 million have a diagnosis of diabetes. This session discusses the innovative multidisciplinary approach to help rural veterans enrolled in the Northern Arizona and Phoenix VA Healthcare Systems manage their diabetes using a virtual platform and health care technology. Presenters will cover how veteran A1c levels improved significantly and the impact of having veterans complete a 3-session technology class prior to wearing an insulin pump, as well as the integration of a Diabetes Self-Management Education and Support program.
