Technology for diabetes management has evolved significantly as insulin pumps and continuous glucose monitors have become advanced, sophisticated, available, and part of the standard of care. The evidence for using diabetes technology in the hospital is scarce and often conducted in a retrospective, uncontrolled manner. It is important to provide education on diabetes technology at the time of admission, during a hospitalization, and from discharge to the first outpatient follow-up visit. This session will focus on the use of diabetes technology during transitions of care and best practices during acute situations, perioperative care, and pregnancy. Based on evidence and clinical expertise, the session provides guidance to attendees for managing devices during transitions of care for people with diabetes.
