Older adults with T2D may have cognitive impairment and functional limitations and may experience the adverse effects of polypharmacy. This session will review deprescribing, medication regimen simplification, and individualized goal setting for older adults taking diabetes, cardiovascular, and/or mental health medications to minimize adverse effects and complications while maximizing therapeutic and humanistic outcomes.  Case vignettes will be utilized to apply concepts and to showcase the role of the DCES in deprescribing and overcoming therapeutic inertia.
