D34 - Apply the Quintuple Aim as a Guiding Framework for Our Specialty’s Transition

The transition from diabetes educator to diabetes care and education specialist recognized the advancement of our specialty practice as part of the changing and dynamic nature of the health care environment. Transition does not occur through a name change alone - our new brand must be lived. Like with the Chronic Care Model, the Institute for Healthcare Improvement’s Quintuple Aim provides a robust framework for certified / diabetes care and education specialists (C/DCES) to focus on and demonstrate their broader value in their respective healthcare environments. The Quintuple Aim’s framework of improving population health, enhancing the experience of care, care team well-being, reducing cost, and advancing health equity will be reviewed, and its application to the role of C/DCESs will be outlined.
