Health disparities are evident in rural communities, especially for people with diabetes. This is complicated by the high prevalence rates of diabetes in this population. Challenges to self-care are multifactorial but start with access to skilled care and education. This session will focus on the use of a CDCES in the role of a care coordinator in a rural primary care practice and the positive impact it has on diabetes management. Case studies will highlight patient barriers and the processes used to increase access to knowledge, skills, medications, and technology in a rural primary care setting, and the subsequent improvement in patient confidence and glucose management.
