UNMC College of Medicine and College of Public Health received a generous 5 million dollar grant from the Diabetes Care Foundation to transform diabetes care and education in Rural Nebraska. Join the presenters as they discuss the selection of rural communities, and the onboarding of local health clinics and clinic champions to help determine areas of focus to improve prediabetes and diabetes outcomes for their patient populations. Examples of interventions and tools utilized in the project include mentorships, dashboards, community coalitions, workforce training, and RN health coach training. Attendees will learn about the progress and shortcomings of their efforts and get a summary of the overall goals and outcomes, as well as an assessment of sustainability and potential to spread across Nebraska.
