This session gives an overview of a project involving clinical pharmacists embedded in clinic settings providing comprehensive medication management to address side effects and drug interactions and assisting in medication taking and intensification therapy. The project evaluated clinical pharmacists and health coaches in a mobile health-driven model of care through a randomized controlled study in urban Black/African American and Hispanic populations with type 2 diabetes (T2D) who were not meeting glycemic targets. Participants in the intervention group received monthly support from health coaches and remote support from clinical pharmacists via video telehealth. Clinical pharmacists and health coaches using mHealth tools improved glycemia among minority participants with T2D who were not reaching their glycemic targets.
