M13 - Leverage an Enhanced Primary Care Model for Diabetes Management in Rural Areas

Primary care practices manage most patients with diabetes and face considerable operational, regulatory, and reimbursement pressures to improve the quality of this care. The Mayo Clinic Health System Enhanced Primary Care Diabetes (EPCD) model was developed by a multi-disciplinary team to leverage the expertise of care team nurses and pharmacists to improve diabetes care and health outcomes, particularly in rural areas with limited access to other resources. In this session, presenters will describe the EPCD model and its implementation across multiple internal medicine and family medicine practices in southeast Minnesota. In addition, they will present evidence of the impact of this model on patient engagement and diabetes management, and discuss lessons learned about challenges, opportunities, and sustainability of team-based models to support diabetes management in primary care.
