A practical equity framework, developed using quality improvement (QI) tools and multidisciplinary stakeholders, can increase equity in diabetes management and outcomes. Using case studies in an interactive format, the session will describe equity adaptations and the use of ten QI tools and an innovative health equity framework. The tools that will be discussed are SMARTER Goals, Run Charts, Failure Mode and Effect Analysis, Prioritization Matrix, Fishbone Diagram, Effort/Impact Matrix, Swim Lane Diagram, Five Whys, SIPOC, and Key Driver Diagram. The presenters will use real-world examples from the T1D ExchangeQI Learning Health Network consisting of 55 diabetes centers.
