The review of glucose data during the clinical encounter and for remote patient monitoring allows for the assessment of glycemic patterns, encourages patient engagement, and fosters a collaborative environment that informs treatment decisions and shared decision-making. In a busy clinical practice with time, financial and staffing constraints, it can be challenging to review glucose data effectively and efficiently in a standardized way. This presentation will highlight a successful approach in leveraging a team-based model to integrate technological tools to optimize the glucose data review process in an ambulatory setting of a large academic medical center.
