S31 - Improve CKD Outcomes by Going Back to the Basics: The DCES-Kidney Care Connection

Almost 90% of adults with T2D and CKD have not been formally diagnosed and therefore, are unaware they have these conditions. This is catastrophic since early diagnosis and management of CKD are essential for preventing significant complications and a decline in quality of life. The DCES is uniquely positioned to significantly impact health outcomes and quality of life for this patient population, regardless of disciplinary background. This presentation will provide practical information for educating patients about kidney disease by going back to the basics while also infusing the nuances that can come with kidney care. This includes differentiating between the various kidney lab parameters, discussing the new race-free eGFR equation, and describing how and when to recommend various nephroprotective agents.
