Food Rx programs (also known as produce prescription programs or fruit and vegetable Rx programs) offer innovative solutions to address the compounding issues for people with diabetes and food insecurity. The National Center for Farmworker Health (NCFH) developed the Food Rx Replication Guide to support health centers to address food insecurity and diabetes incidence through a model that seeks to build partnerships between the local food system, the healthcare system, and the patient population. This presentation will familiarize attendees with the Food Rx Replication Guide and demonstrate how to use it to create their very own personalized produce prescription programs to improve diabetes outcomes in partnership with local organizations.
