Evidence shows that health disparities in chronic disease management persist in the geriatric population. This session will give an overview of a community-focused program in senior housing facilities offering clinical guideline-driven, chronic-disease-management recommendations and education. The program trains and empowers seniors to voice their healthcare-related concerns better when communicating with their health care providers. Attendees will learn a patient-centered approach to use that reduces health disparities in this population through an illustrative case presentation.
