Inadequate access to diabetes self-management education and support (DSMES) is a major barrier to optimal diabetes care, contributing to poor health outcomes, particularly among rural residents and other underserved populations. Community paramedics are experienced emergency medical services providers who are additionally trained in chronic disease management, primary and preventive care, and social determinants of health. They are uniquely positioned to deliver comprehensive person-centered care, particularly to clinically and psychosocially complex patients and those who live in remote or underserved areas. In this session, we describe the development, implementation, and evaluation of a community paramedic program focused specifically on caring for patients with unmanaged diabetes (those with elevated hemoglobin A1c or experiencing severe hypoglycemia) across rural southeast Minnesota.
