F02 - Intervene to Reduce Risks Associated with Osteoporosis, Falls, and Fractures

When DCESs engage with a person with diabetes, we routinely consider complications and co-morbidities impacting the individual. We aim to identify problems and implement interventions to prevent or delay potential negative sequelae. Osteoporosis falls, and fractures are associated with diabetes and should be considered along with other complications. DCESs can play a key role in the identification of a person with, or at risk of, osteoporosis and fracture. This presentation will describe the scope of the problem; review possible causes for falls and fractures; guide the DCES in identification and interventions to decrease or prevent falls and fractures; and explore opportunities for referrals and community resources to decrease complications related to osteoporosis, falls, and fractures for an individual with diabetes.
