In order to deliver the best care for emerging adults with T1D, we describe the steps for creating a Young Adult Diabetes Knowledge Assessment tool including a review of the literature on transition and diabetes, input from focus groups of patients who had successfully transferred diabetes care from pediatric to adult endocrine specialists, and an evaluation of the alignment of the tool with the requirements for ADA Recognition. Based on 3 years of use in a busy clinic, knowledge strengths and deficits of this population were identified, which led to a shorter assessment tool that was based on the input from focus groups of those delivering, as well as those receiving care.
