Providing diabetes care and education to those with moderate to severe mental illness presents many challenges. This session will review tips and tricks used by a multidisciplinary team in a community-based psychiatric health facility to support people with schizophrenia and other severe mental illnesses with a dual diagnosis of diabetes.  Using a population health-based model, attendees will learn how this team combines services to reach those who have historically not received care for their diabetes. The presenter will share strategies used to provide education and support with 3 unique case studies.
