Severe insulin resistance is defined as total daily insulin requirements exceeding 200 units per day or greater than 2 units/kg/day. Patients with severe insulin resistance may benefit from concentrated insulin such as U-500 due to impaired absorption of U-100 at higher doses. Clinical inertia and safety concerns have led to provider reluctance to utilize U-500 insulin in patients in whom this therapy is indicated. It is important to individualize pharmacotherapy recommendations in patients with severe insulin resistance to prevent diabetes-related complications. This session will discuss the properties of U-500, identify ideal candidates, and review important safety considerations with a focus on initiation and titration. Interactive patient cases will be used to illustrate the application of new clinical knowledge on this topic.
