Describe the process of working through a health system across multiple disciplines to implement new technologies to address the needs people with diabetes require to be successful from basic diabetes management to advanced management tools. Identify a new model of diabetes management in the outpatient arena. Discuss challenges met, barriers and billing and reimbursement options. Describe the project management process and identifying key stakeholders.
